Victorian Cardiac Clinical Network

 

Description

The Victorian Cardiac Clinical Network is a group of health professionals, health organisations and consumers who work together to promote continuous improvement in cardiac care in Victoria. The Network supports the consistent application of evidence-based practice, measures clinical performance and strengthens relationships and referral pathways between and within health services.

The Network includes cardiac rehabilitation and acute coronary syndrome subcommittees, and has appointed seven regional cardiac facilitators. It is guiding the implementation of the key strategies in Victoria's 5-year cardiac plan, Heart health: improved services and outcomes for Victorians (2015).
