Assessment and Care Planning in Rural, Remote and Indigenous Communities

 

Overview

Target audience: Health professionals and other workers who provide assessment and care co-ordination for the elderley, people with disabilities, people with mental health issues and other vulnerable clients in the community

Course summary: This one day workshop will build skills for those who undertake this important role and enhance appropriate care for these vulnerable and disadvantaged people. The course provides an opportunity for interactive and inter-professional learning based around the use of case studies and other activities.  The course was developed in conjunction with the National Aging Research Institute.

At the end of this workshop the participants will be able to:
